An upcoming webinar hosted by Resources for Integrated Care (RIC) will examine the challenges associated with traditional care silos and their impact on comprehensive care coordination. This webinar will focus on strategies to improve behavioral health condition management in persons with I/DD who are dually eligible for Medicare and Medicaid. Speakers will discuss the importance of person-centered, holistic care coordination approaches and explore how health plans can bridge gaps in existing systems by utilizing interdisciplinary care teams, integrating community-based organizations, and promoting family and caregiver education and support to meet the ongoing needs of dually eligible individuals.
