
On August 1, 2023, the Department of Health will transition the LTC RISE program to its next phase: LTC RISE 2.0! In the next iteration of the program, the Department aims to increase the scale of identified best practices from each region of the program across the state. The most promising quality improvement activities from RISE 1.0 were identified and organized into 6 themes:
- Leadership improvement
- Frontline staff assessment and skill building
- Infection prevention and control
- Immunizations
- Emergency preparedness
- Resident care
These quality improvement activities offered regionally in LTC RISE 1.0 are recommended to be offered statewide in LTC RISE 2.0, working to ensure that all facilities in the state may benefit from these opportunities. Facilities should contact their regional RISE partner to learn more about newly offered initiatives. Facilities who have not yet enrolled in LTC RISE are encouraged to do so by contacting their regional RISE partner.
In addition, the program will be scaling back on crisis response to COVID-19 infection activities, however preserving the capabilities to respond in the event of ongoing COVID-19 outbreaks.
