ODP and OVR invite provider agencies enrolled to deliver Supported Employment, Advanced Supported Employment/Customized Employment, Pre-Employment, or Transition Services to complete a survey. Information gathered from the survey on staffing, training, service options, and coordination may influence the provision of employment services funded by ODP and/or OVR for people with disabilities. The survey is completely voluntary, all responses will be anonymous and the survey should take about 10 minutes to complete. The survey should be completed by provider staff responsible for
hiring, training, and programmatic decision-making.
